Caregivers provide essential services such as feeding, walking, and companionship, ensuring that pets receive the attention they need. With regular updates and communication from the caregiver, pet owners can enjoy their time away, knowing that their furry friends are safe and comfortable in their own space. This is a great option for owners who have multiple pets, of different kinds.

understand that it is never easy leaving behind your furry loved one, so we will make every effort to provide quality care

and services while your pet is here with us. The safety and well‐being for both your pet and our employees is our top

priority. It is important to us that you understand and follow our policies so that you and your pet have a great

experience at Paws & Relax Pet Care LLC. Please take this time to review our policies and acknowledgements:

Vaccination Requirements

Pet owners must provide proof of vaccinations at least one day prior to your pet coming to Paws & Relax Pet Care. All dogs must be up to date and fully vaccinated by a licensed veterinarian for the following: ‐ 1‐ or 3‐year Distemper/Parvo ‐ 1‐ or 3‐year Rabies

Aggressive Behavior Acknowledgment

Owners must inform Paws & Relax Pet Care LLC if your pet has ever bitten or shown signs of aggressive behavior towards another dog or person. In the event of aggressive behavior, Paws & Relax Pet Care LLC will make every effort to take care of your pet but under no circumstances will we put our staff or other guests/dogs in danger. This applies to all services rendered including boarding, daycare, and grooming. Paws & Relax Pet Care LLC reserves the right to terminate services at any time if a pet cannot be safely handled at our facility. If your pet were to cause bodily harm to a person or other dog, as owner you agree to be responsible for all related medical bills and recovery costs to all injured parties.

Emergency Release and Medical Treatment Authorization:

In the event that your pet needs medical attention, Paws & Relax Pet Care LLC will try to contact you before contacting your veterinarian. If for any reason we cannot get in touch with neither you nor your veterinarian, a different vet will be contacted so that your pet receives the medical attention it needs. Owner agrees to and gives consent to all treatments and pay all costs associated with such medical treatment.

Medications & Supplements Acknowledgement

Any medications or supplements for your pet must be in their original packaging, including any prescription bottles, to be administered to your pet by Paws & Relax Pet Care LLC. Owner gives permission to Paws & Relax Pet Care LLC staff to administer medication to your pet based solely on medication instructions provided on registration form. Only medication and supplements that can be eaten by your pet during their regular feeding time will be administered. No other forms of medication, including injections, will be done. Paws & Relax Pet Care LLC will not be responsible for any liability arising from medication your pet refuses to take.

Feeding Acknowledgement

Owner is strongly encouraged to provide Paws & Relax Pet Care LLC with food for their pet during their stay. Food should be labeled with pet name and meal amount (ex., “Molly 1 cup AM and 1⁄2 cup PM), or food can be separated into individual baggies marked with name and mealtime (AM or PM). In the event owner’s pet does not have food or runs out of food, owner gives consent to Paws & Relax Pet Care LLC to provide food to pet. A fee of $5 per day will be added to the invoice for food provided by Paws & Relax Pet Care LLC. Any pets that cannot eat together will not be allowed to stay together.

Play Time/ Outdoor Activies Acknowledgement

Paws & Relax Pet Care LLC will make every effort to provide a safe and playful interaction with your pet(s) at your home. During this time the staff at Paws & Relax Pet Care LLC will closely supervise all play, but it is possible that unknown altercations or aggressive behavior can occur between pets. Owner gives consent to staff members of Paws & Relax Pet Care LLC to use designated areas for playtime at Owner’s residence, and secure or put away any items that could cause harm to pet(s) or damage to residence. Owners who give consent to playtime for their pet(s) will assume all risks and take full responsibility for any related medical costs to their pet associated with this activity and not hold Paws & Relax Pet Care LLC and its staff liable for any damages. In addition, dogs who are taken for walks must have a collar or body harness available for use by pet, otherwise Paws & Relax Pet Care LLC reserves the right not to walk the pet. Pets will remain in designated area for walking by staff member of Paws & Relax Pet Care LLC. In the event a situation shall arise that does not seem safe for either the pet and/or the staff member, the walk will end immediately, and the pet returned to the Owner’s residence. Any fees associated with this service are still owed.

Fees & Payment Acknowledgement

At Paws & Relax Pet Care LLC, we strive to provide high quality, professional services for our customers and their pets. It is important to us that we make every effort to create a safe and reliable experience for our customers and their pets while visiting their home for pet care services. To ensure that time spent with our customers is valuable, we will require a deposit of at least 50% for services agreed upon with Owner. Any services not paid for after 7 days of completion will result in a $25 late fee for each day thereafter payment is past due. Owner agrees to pay Paws & Relax Pet Care LLC for all services rendered.

Reservation Cancellations/No-Shows Acknowledgment

At Paws & Relax Pet Care LLC, we understand things can come up where you need to modify or cancel your pet’s boarding dates. We ask that you please provide us with at least 24 hours’ advance notice‐ any cancellations made within 24 hours will be subject to the full amount due for next day services. If Owner does not provide Paws & Relax Pet Care LLC with a reliable source to enter owner’s residence to care for pet(s), such as a key or lock code, for the date(s) in which services are to be rendered, owner will be billed for services regardless and owner agrees to pay for these services.

I certify that I am the owner of record for the pet(s) described on the Paws & Relax Pet Care LLC registration form. As such, I am both legally and financially responsible and am authorized to make all necessary decisions on behalf of such pet. I give permission and consent to Paws & Relax Pet Care LLC and its staff members to take care of my pet(s) while in their care. I agree to pay for all services rendered by Paws & Relax Pet Care LLC in full before my pet(s) will be released. I give consent up to 12 months beyond date of signature below.
